1. Information Collection & Use
You must submit certain information with HomeHealthNYC.com to receive HomeHealthNYC.com’s home health care provider referral services. To make your submission, you must complete the form contained on our website. In connection with the submission, you are required to provide certain personal information, including your name, address, email address, telephone number, and information about your home health care related needs. This information will only be used to provide you with the Services.
We may send you service-related announcements from time to time for quality assurance purposes. If you do not wish to receive such emails, you have the option from within such emails to opt-out from future receipt.
Based on the personally identifiable information you provide us, we will also communicate with you in response to your inquiries, to provide the services you request, and to manage your submission. We will communicate with you by email unless you request to be contacted by telephone.
Usage Statistics and other Non-Personally Identifiable Information
As is true of most websites, we may gather, analyze and store certain non-personally identifiable information automatically. For example, we may use Google Analytics, which is a tool offered by Google for analyzing website visitor statistics. This information may include, but is not limited to, internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and clickstream data. We share aggregated demographic and website usage information about our users and user base with our business partners, such as companies that advertise products and services on our website. This information does not identify individual users.
Personally Identifiable Information
We do not share personally identifiable information with third parties except for the purpose of providing the Services. For example, we will provide your name, address, other contact information, and your Survey responses to home health care service providers that we think you may want to consider for your home health care related needs. Although we inform such service providers of the confidential nature of personally identifiable information that we send to them, we cannot control or be responsible for how such providers may use or disclose such information once it is received.
Disclosures Required or Permitted by Law
We reserve the right to disclose your personally identifiable information as required by law, when we believe that disclosure is necessary to protect our rights, to comply with a judicial proceeding, court order, or legal process served on us, and/or as permitted by applicable law.
If HomeHealthNYC.com merges with or is acquired by another company or sells some or all of its assets, HomeHealthNYC.com has the right to transfer your personally identifiable information in connection with any such merger, acquisition, asset transfer or other similar business transfer.
3. Data Retention; Access to Personally Identifiable Information
Upon completion of providing you with the Services, we will delete from our data bases all of the information you have provided to us, except for the following information: name, mailing address, telephone number, and email address (“Retained Information”). You may delete your Retained Information as described in the paragraph below.
If your Retained Information changes, or if you no longer desire our services, you may correct, update, or delete your Retained Information, including the Retained Information, by emailing your request to us at the email address listed below in Section 6.
When you enter sensitive information as part of your user submission, we encrypt that information using secure socket layer technology (SSL) or other similar commercially reasonable encryption technology. We use commercially reasonable efforts to protect the personal information submitted to us, both during transmission and once we receive it. However, no method of transmission over the Internet, or method of electronic storage, is 100% secure. Therefore, while we strive to use commercially reasonable means to protect your personal information, we cannot guarantee its absolute security.
