(referred to below as “HomeHealthNYC.com”, “us” or “we”), the owner of the website located at HomeHealthNYC.com, respects your privacy and is committed to respecting your personal information in accordance with this Website Privacy Policy.  By using this site, you consent to the collection and use of information as set forth in this Privacy Policy.
We are not a provider of medical advice, assessment or treatment; therefore, we are not a Covered Entity under the Health Insurance Portability and Accountability Act (HIPAA), nor are we a Business Associate under HIPAA.  Nevertheless, you consent to our use and disclosure of your personal information as provided in our Terms of Use and this Website Privacy Policy.

1. Information Collection & Use
User Submission
You must submit certain information with HomeHealthNYC.com to receive HomeHealthNYC.com’s home health care provider referral services.  To make your submission, you must complete the form contained on our website.  In connection with the submission, you are required to provide certain personal information, including your name, address, email address, telephone number, and information about your home health care related needs.  This information will only be used to provide you with the Services. 

Service-related Announcements

We may send you service-related announcements from time to time for quality assurance purposes.  If you do not wish to receive such emails, you have the option from within such emails to opt-out from future receipt.

Customer Service

Based on the personally identifiable information you provide us, we will also communicate with you in response to your inquiries, to provide the services you request, and to manage your submission.  We will communicate with you by email unless you request to be contacted by telephone.

Usage Statistics and other Non-Personally Identifiable Information

As is true of most websites, we may gather, analyze and store certain non-personally identifiable information automatically.  For example, we may use Google Analytics, which is a tool offered by Google for analyzing website visitor statistics.   This information may include, but is not limited to, internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and clickstream data.  We may store such non-personally identifiable information that we collect

We share aggregated demographic and website usage information about our users and user base with our business partners, such as companies that advertise products and services on our website. This information does not identify individual users. 

Personally Identifiable Information

We do not share personally identifiable information with third parties except for the purpose of providing the Services.  For example, we will provide your name, address, other contact information, and your Survey responses to home health care service providers that we think you may want to consider for your home health care related needs.  Although we inform such service providers of the confidential nature of personally identifiable information that we send to them, we cannot control or be responsible for how such providers may use or disclose such information once it is received.

Disclosures Required or Permitted by Law

We reserve the right to disclose your personally identifiable information as required by law, when we believe that disclosure is necessary to protect our rights, to comply with a judicial proceeding, court order, or legal process served on us, and/or as permitted by applicable law.

Business Transfer

If HomeHealthNYC.com merges with or is acquired by another company or sells some or all of its assets, HomeHealthNYC.com has the right to transfer your personally identifiable information in connection with any such merger, acquisition, asset transfer or other similar business transfer. 

3.  Data Retention; Access to Personally Identifiable Information

Upon completion of providing you with the Services, we will delete from our data bases all of the information you have provided to us, except for the following information:  name, mailing address, telephone number, and email address (“Retained Information”).  You may delete your Retained Information as described in the paragraph below. 
If your Retained Information changes, or if you no longer desire our services, you may correct, update, or delete your Retained Information, including the Retained Information, by emailing your request to us at the email address listed below in Section 6.

4.  Security

When you enter sensitive information as part of your user submission, we encrypt that information using secure socket layer technology (SSL) or other similar commercially reasonable encryption technology.  We use commercially reasonable efforts to protect the personal information submitted to us, both during transmission and once we receive it. However, no method of transmission over the Internet, or method of electronic storage, is 100% secure. Therefore, while we strive to use commercially reasonable means to protect your personal information, we cannot guarantee its absolute security.

5.  Changes to this Website Privacy Policy
If we decide to change our Website Privacy Policy, we will post those changes to this document and any other places we deem appropriate so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it.

We reserve the right to modify this Website Privacy Policy at any time, so please review it frequently. If we make material changes to this policy, we will list such changes below.
